Can you bill me for my copay?
Your copay is due at the time services are rendered.

How do I obtain a copy of my medical records?
You must sign a medical release form. If your records are being sent to anyone other than a physician or your current insurance company, there is a charge of $20.00 for the first 40 pages and 25¢ for each additional page after 40.
How do I get a current prescription refilled?
We ask that you contact your pharmacy. They will forward a request for your prescription refill. Please allow 24 hours for the request to be complete.
